Patient description: Medically stable – able to actively participate with, and benefit from, therapy. Will include spectrum of initial severity of injury with a small minority derived from Minor HI category.
Description of rehabilitation input: Interdisciplinary co-ordinated management therapy aimed at community re-integration/inclusion by enhancing independence, wellbeing, & assist return to work/education. In collaboration with Social Services, neuropsychiatry, voluntary and statutory services. Includes treatment of patients in residential care or with live-in carers.

Barking and Dagenham – Community Rehabilitation Service

Rehabilitation for adults in a community (home-based) setting for those with a neurological or musculoskeletal condition who require home-based therapy and rehabilitation. Therapy is provided to patients predominantly within their own home and there is also access to a gym at Porters Avenue Health Centre.

The service is for people over 18 years old, excluding those requiring rapid response intervention, maintenance therapy or those who only need major adaptations to the home environment and have no other therapy needs

The Bromley SCRehN team is a specialist community neurological rehabilitation service for Bromley adult residents. We work with clients with a neurological diagnosis such as stroke, brain injury, Parkinson’s disease, Multiple Sclerosis and Spinal injury.

The team is made up of experienced physiotherapists, occupational therapists, speech and language therapist, a clinical nurse specialist, a dietitian, a clinical psychologist and therapy assistants

They provide rehabilitation in patient homes for periods of 6–12 weeks to help increase independence through a personalised treatment programme.

CHOICES (Client-centred, Holistic, Occupational therapy promoting Independence in the Community, which is Enabling and Supportive).

Assessment, treatment and ongoing support for people with physical disabilities who are experiencing difficulty getting on with their daily lives. The team will:

assess clients in their own environment focusing on productivity, leisure and self-care.
consider people’s activities and feelings which are important to their independence
plan, set-up and monitor individual treatment programmes agreed with clients and carers to help re-integrate them into society.
prescribe equipment and advise on issues in the local environment which may affect the individual client.

Lambeth – Neurological Rehabilitation Team

In April 2011, Lambeth Community Health and Southwark Provider Services merged and integrated with Guy’s and St Thomas’ Hospital Foundation Trust. The Community Health Services are now known as “Guy’s and St Thomas Community Health Services”. Within these services, the Neuro Rehabilitation Pathway aims to work in partnership with adults over 16 years with an acquired neurological diagnosis who live in the two boroughs, to enable them to achieve and maintain maximum possible function and independence through multidisciplinary community-based specialist neurological rehabilitation based on personal goals.

The Lambeth team consists of Speech and Language Therapists, Physiotherapists and Occupational Therapists. They provide a service for adults and older people with illness, injury or disability.

They see people at a local health centre or at their home if this is more appropriate. Sometimes it may be more suitable to work with people in other settings such as at work or at college.

They have an open referral system which means anyone can refer to their service. They can be contacted by telephone on 020 3049 4004
